Dress Code Policy
I understand and acknowledge that, in my position with the Company, I am required to abide by the rules set forth in the BrandsMart U.S.A. Dress Code Look Book (“Dress Code”) with regard to my clothing choices. If the Dress Code requires me to wear a BrandsMart U.S.A. shirt, I acknowledge that the Company has provided me with a reasonable and sufficient number of such shirts free of charge. If the Dress Code does not require me to wear a BrandsMart U.S.A. shirt or if I wish to obtain more BrandsMart U.S.A. shirts, I may choose to purchase and wear to work such BrandsMart U.S.A. shirts even though I am not required to do so. I specifically acknowledge and agree that I am in no way expected or required as a condition of my employment to purchase any BrandsMart U.S.A. shirts or any other BrandsMart U.S.A. attire.

Payroll Deduction Policy
If selecting to utilize the payroll deduction option as my payment method, then I request and do hereby authorize Interbond of America, LLC d/b/a BrandsMart USA to deduct and withhold the full purchase price of my voluntary apparel and/or accessories purchase as a single deduction from my wages and other compensation arising from my employment with the Company to which I may now or hereafter be entitled. I further agree that, in the event my employment shall terminate, either voluntarily or involuntarily, prior to the full repayment of the total amount of the purchase, the Company may withhold the remaining amount owed from my final pay, except to the extent prohibited by federal or state minimum wage law. I represent that this authorization is executed voluntarily and has not been made as a condition of my continued employment.
